About this resource
Delivers a variety of support services for individuals facing intellectual and/or developmental disabilities. These services encompass case management, personalized support plans for individuals and families, access to brokerage services, in-home assistance or residential placements, employment and vocational training, as well as abuse investigation and protective services.
Intake Procedure: To initiate the process, call to obtain an application for Developmental Disabilities services. An eligibility worker will schedule an appointment to assist you with the application process.
Fees: There are no fees associated.
Who can use this
- Individuals with an intellectual or developmental disability who are residents of Oregon and U.S. citizens.
Documents you may need
- Proof of citizenship, proof of address, and records of any/all psychological and medical assessments related to the disability.
