This initiative delivers confidential case management and independent living services tailored for individuals with HIV. It offers financial support for HIV medications, transportation to medical appointments, insurance premium coverage, transitional housing, rent assistance, dental care, disease management education, and referrals to other agencies for essential needs, employment, and substance abuse treatment.
