This program provides various support services such as case management, personalized support plans for individuals and families, brokerage services for transportation, in-home assistance or residential options, and employment and vocational training.
Intake Process: To begin, call to request an application for Developmental Disabilities services. An eligibility worker will set up an appointment to guide you through the application process.
Location: Situated on the Heights near 13th and Pacific Avenues.
Cost: There are no fees associated with these services.
Who can use this
- Eligibility is for individuals with an intellectual or developmental disability who are residents of Oregon and US citizens.
Documents you may need
- Proof of citizenship, proof of address, and records for all psychological and medical assessments related to the disability
