About this resource
Patients receive individualized support from a Certified Community Health Worker and a Registered Nurse throughout their pregnancy journey. The program includes prenatal and postpartum care, education regarding pregnancy, and assistance with breastfeeding and breast pump usage. Additionally, it provides mental health services and family planning support.
Intake Procedure: Schedule an appointment via phone.
Fees: Accepts OHP, Medicare, and private insurance, with a sliding scale discount available for those without insurance.
