This program delivers assistance to those facing intellectual and/or developmental disabilities. The services offered encompass case management, tailored support plans for individuals and families, brokerage services for transportation, in-home assistance or residential options, as well as employment and vocational support.

Fees: There are no charges for these services.
Who can use this
- Individuals with intellectual or developmental disabilities who are U.S. citizens.
Documents you may need
- Proof of citizenship, proof of address, and records of any/all psychological and medical assessments related to the disability.
