About this resource
This program delivers a variety of support services for those with intellectual and/or developmental disabilities. Services offered may include case management, personalized support plans for individuals and families, brokerage services for transportation, in-home assistance or residential options, as well as employment and vocational support.
Intake Procedure: Schedule an appointment by phone.

- Individuals who have an intellectual or developmental disability, must be residents of Oregon and U.S. citizens.
