Intake Procedure: Schedule an appointment via phone.
Travel/Location: Situated on 12th Street, directly opposite St Charles Madras hospital.
Fees: Accepts OHP, Medicare, various private insurance plans, or payments made out of pocket. A 20% discount is available for patients without insurance.
