Mosaic Community Health

This organization provides an extensive array of primary and preventive health care services to the community. These services encompass general medical examinations, annual women's health check-ups, STD testing and treatment, pediatric services, vaccinations, mental health assessments and counseling, as well as evaluations for drug and alcohol use. Additionally, it assists individuals with enrolling and re-enrolling in the Oregon Health Plan (OHP).

Travel/Location: The clinic is situated within the South Y Complex, located at the back of the building on the ground floor. Access the clinic parking lot via K Street or L Street leading to 3rd Street.

Fees: The clinic accepts OHP, Medicare, and most private insurance plans. There may be sliding scale fees and payment plans available.
