About this resource
This service delivers comprehensive medical care that encompasses diagnosis and treatment for illnesses and injuries, routine examinations for infants and children, immunization services, podiatry, pap smears and breast health check-ups, as well as on-site laboratory and x-ray facilities. It also provides information regarding the Special Diabetes Program for Indians (SPDI), access to pharmacy services, and potential referrals to off-site medical specialists.
Intake Procedure: Schedule an appointment by phone.
Fees: Charges vary depending on the service provided. They accept OHP, Medicare, and other insurance plans, and also offer a sliding scale fee structure.
Who can use this
- Individuals who are Native American and belong to federally recognized tribes.
Documents you may need
- Tribal card or certification from a federally recognized tribe, birth certificate/photo identification such as state issued ID or a driver's license, social security card, insurance cards. Proof of income will be required for those without insurance. Klamath Tribal members or children of Klamath Tribal members residing in Kalmath County must provide a current utility bill or rental receipt with rental contract in order to show residency.
