About this resource

This program delivers in-home care services that encompass housekeeping, personal assistance, laundry, shopping, transportation, and meal preparation and delivery. It also provides adult day services, which are non-residential community-based programs catering to individuals for part of the day. Registered nurse services are available for one-time or intermittent assessments, service planning, medication monitoring, provider training, and education. Additionally, the program includes case management and options counseling to help individuals make informed decisions regarding services based on their strengths, needs, and preferences. The number of service hours per person varies based on assessed needs and available funding, typically ranging from 5 to 20 hours per month. Importantly, benefits are not subject to estate recovery.

Who can use this
