This program offers various support services including case management, family support plans, brokerage services, in-home assistance or residential options, vocational services, abuse investigations, and protective services.
Intake Process: Contact us to schedule an intake appointment. An eligibility specialist will assist with the application process.
Cost: No fees are charged.
Who can use this
- Eligible individuals include those with an intellectual or developmental disability, who are residents of Oregon and U.S. citizens.
Documents you may need
- -Proof of citizenship and Oregon residency -Proof of address -Any records of psychological and medical assessments related to the disability
