About this resource
Case managers collaborate with clients who are HIV-positive to formulate care plans that enhance their self-care capabilities. The services provided may encompass medical and pharmacy case management, respite care, in-home support, advocacy for benefits and health insurance, meal provisions, and emergency financial aid. Additionally, referrals can be made for social services, housing, dental care, mental health and addiction treatment, employment support, and financial planning.

Fees: No charges apply.
Who can use this
- This program is available for individuals living with HIV, with emergency financial aid restricted to those earning below 250% of the federal poverty level.
