About this resource
This initiative offers case management services to assist households in accessing resources and locating housing. Households that qualify are evaluated and supported through the OHP Health-Related Social Needs benefit. There may also be opportunities for extra rent and utility assistance depending on eligibility and available funding.
Intake Procedure: Contact to schedule an appointment, which can be conducted via phone or video call only.
Travel/Location: Located on 5th Avenue between Adams Street and Blair Boulevard, with a Lane Transit District (LTD) bus stop just a block away.
Fees: There are no fees associated with this service.
Who can use this
- Families with children under the age of 18.
