About this resource

This service delivers comprehensive primary health care, mental health assessments, and counseling. It encompasses routine physical examinations, women's health services, behavioral health support, diabetes management, and various therapies, including physical therapy, specialty care, and referrals to specialists, respiratory therapy, and smoking cessation assistance. An on-site pharmacy provides some medications at no cost to patients.

Who can use this

  • - Must be 18 years or older - Gross household income between 85%-300% of the Federal Poverty Level (FPL) - Uninsured individuals meeting Volunteers In Medicine criteria - Those not eligible for state-funded healthcare - Oregon Health Plan and Medicare patients who lack a provider or cannot obtain one - Insured individuals with a deductible of $2,500 or higher - Medical issues not resulting from a motor vehicle accident or work-related injuries covered by Workers' Compensation.
