About this resource
Provides a variety of support services for individuals with intellectual and/or developmental disabilities. These services may include case management, personalized support plans for individuals and families, access to transportation brokerage services, in-home assistance or residential options, as well as employment and vocational support.
Intake Process: To begin, call to request an application. An eligibility worker will set up an appointment to guide you through the application process.
Costs: No fees are required.
Who can use this
- Individuals with an intellectual or developmental disability who are residents of Oregon and US citizens.
Documents you may need
- Proof of citizenship, proof of address, records of all psychological and medical assessments related to the disability
