Intake Procedure: Schedule an appointment by phone.

Travel/Location: The clinic is situated between Queen Avenue and Pacific Highway. From Highway 99E (Pacific Boulevard), turn right at the Human Bean coffee shop, and the clinic is in the large building on the left after United Way of Linn County. There is a bus stop at the corner. For those coming from I-5 South, take Exit 234-A, follow signs to Corvallis, and turn left on Hill Street. If coming from I-5 North, take Exit 233, turn left onto Highway 20, follow signs to Corvallis, and then turn left on Hill Street.

Fees: No charges apply.

Who can use this

  • Children and teenagers aged 1-18 without dental insurance or with specific OHP dental coverage.
