About this resource
Delivers a variety of support services tailored for individuals facing intellectual and/or developmental disabilities. These services may encompass case management, personalized support plans for individuals and families, access to brokerage services for transportation, in-home assistance or residential options, as well as employment and vocational training.
Intake Process: To initiate, call to obtain an application for Developmental Disabilities services. An eligibility worker will arrange an appointment to guide you through the application steps.
Costs: There are no fees involved.
Who can use this
- Individuals with an intellectual or developmental disability who are Oregon residents and U.S. citizens.
Documents you may need
- Proof of citizenship, proof of address, and records of any/all psychological and medical assessments related to the disability.
