This initiative involves public health nurses (PHNs) visiting homes to collaborate with families in order to enhance health outcomes related to pregnancy, family, and child well-being. It offers services such as health and developmental screenings, nutrition and health education, advice on infant and child development, breastfeeding assistance, and connections to community resources tailored to the family's needs.

Who can use this
- Expectant individuals, infants and children aged 0-5, along with parents or primary caregivers who are at greater risk for health challenges, including factors like prematurity, teenage parenting, domestic violence, and drug exposure.
