About this resource
Delivers a variety of support services tailored for individuals facing intellectual and/or developmental disabilities. These services encompass case management, creation of individual and family support plans, access to brokerage services for transportation, in-home assistance or residential placement, as well as employment and vocational support.
Intake Procedure: To begin, call to obtain an application for Developmental Disabilities services. An appointment will be arranged with an eligibility worker who will guide you through the application process.
Fees: There are no associated fees.
Who can use this
- Individuals with an intellectual or developmental disability who are Oregon residents and U.S. citizens.
Documents you may need
- Proof of citizenship, proof of address, records of any/all psychological and medical assessments related to the disability
