This program delivers a comprehensive range of support for individuals dealing with mental health challenges, focusing on guiding them towards optimal recovery and successful integration into community life. The services provided encompass individual and group therapy, skill enhancement, targeted case management, assistance with hospital discharge, dual diagnosis treatment, peer support services, representative payee services, residential care, 20-hour personal care, and coordinated care services. The majority of these services are offered at the client's home.

Fees: Charges vary depending on the service, with sliding scale options available. Support for reviewing income and insurance coverage is also provided.
