About this resource
Health-related social needs (HRSN) refer to fundamental requirements impacting a person's health. These benefits aid eligible members with essential services such as assistance with rent and utility payments, tenant support, and storage fees. HRSN housing services are available to members only once throughout their lifetime.
Intake Procedure: For CCO members, contact the CCO or check the website for details. Open card members can call, email, or visit the website for assistance. Clients not enrolled in a CCO should call for information.
Fees: There are no fees associated with these services.
Who can use this
- Eligible OHP members must be either 65 years or older, children aged 0-5, pregnant or have given birth in the last year, or have a qualifying health condition, with a household income at or below 30% of the local average, and must need housing support.

- -Medicaid ID number, Rental/lease agreement, or utility bill
