About this resource
Health-related social needs (HRSN) refer to fundamental requirements that impact an individual's health. These benefits assist eligible members with essential needs such as help with rent payments, utility bills, tenant support, and storage costs. Each member is entitled to receive HRSN housing services only once throughout their lifetime.
Intake Process: For CCO members: Contact the CCO or check the website for details. For open card members: Reach out via phone, email, or visit the website for assistance. For clients not enrolled in a CCO: Please call for information.
Costs: There are no fees associated with this service.
Who can use this
- Eligible OHP members must be either 65 or older, children aged 0-5, pregnant or have given birth in the last year, or have a qualifying health condition, and must meet specific income and housing requirements.

- -Medicaid ID number, Rental/lease agreement, or utility bill
