About this resource
This service provides in-home assistance, encompassing tasks such as housekeeping, personal care, laundry, shopping, transportation, and meal preparation and delivery. It also includes adult day services, which are community-based programs that operate for part of the day. Registered nurse services are available for one-time or occasional assessments, service planning, medication monitoring, and provider training and education. Additionally, case management and options counseling are offered to help individuals make informed decisions about services based on their strengths, needs, and preferences. The amount of service hours provided is determined by individual assessments and available funding, and benefits are exempt from estate recovery.
Intake Procedure: Interested individuals can call or visit the website for further details. Please note that some services may have a waitlist.
Travel/Location: The facility is situated in a two-story building just north of the Cherry Avenue NE and Salem Parkway intersection, with the entrance being the first one north of Salem Parkway on the east side of Cherry Avenue.
Fees: There are no fees associated with these services.
Who can use this
- Adults aged 18 and older who require assistance with personal care, have an income below 400% of the Federal Poverty Level, possess assets below the average cost of six months in a nursing facility, and reside in their own home.
