Application Steps: - Submit an application online - Send an email with the completed application attached. Please note that information may not be secure if sent without a secure email method. - Alternatively, you can mail, fax, or deliver the completed application to your local office. The application form is available in multiple languages including English, English Large Print, Spanish, Spanish Large Print, Arabic, Burmese, Nepali, Russian, Somali, and Vietnamese.
