About this resource
This program delivers adaptable, family-oriented, and culturally aware case management services to support families in managing the care of their children with significant disabilities within their homes. Services are tailored to meet the specific needs of both the child and the family while ensuring efficient use of state and federal resources. CIIS features three specialized sections designed to cater to the requirements of children with disabilities, allowing a child to qualify for services through the Intensive Behavior, Medically Involved, or Medically Fragile Programs.

Who can use this
- Eligibility varies by program: Most services are available for individuals aged 17 and younger, extending up to age 21 if program criteria are fulfilled.
Documents you may need
- Birth certificate Social Security card Health Insurance card (back and front) Proof of child income (SSI, child support, etc.) Copy of any resources in the child's name (bank statements, trusts, etc.)
