About this resource
Nutrition Support is a health-related social need (HRSN) service offered by the Oregon Health Plan aimed at aiding members with health-related conditions that can benefit from nutritional assistance. This service may encompass: Medically Tailored Meals (MTMs): -Meals can include fresh or frozen options -Up to three meals daily for a maximum of six months -Meals are delivered to the Member's home or private residence, which may include shelters that do not serve meals. Nutrition Education: -Offers evidence-based nutrition education to help individuals make healthier food choices and adopt beneficial food-related behaviors, while considering personal dietary preferences and cultural appropriateness. Pantry Stocking: -Provides a portion of the recommended daily food intake for up to six months, with options including fresh, frozen, dried, pureed, or canned foods. Eligible members will receive this benefit as a grocery box delivered to them or through a pre-loaded card for use at select grocery stores. Fruits and Vegetables: -Available for up to six months, including fresh, canned, packaged, frozen, dried fruits and vegetables, and fresh herbs. Eligible members will receive this benefit similarly as a grocery box or a pre-loaded card.
Intake Procedure: For CCO members: Call the CCO or visit their website for more details. For open card members: Call or email for information. For clients not in a CCO: Call for assistance.
Fees: There are no fees associated with this service.
Who can use this
- OHP members facing food insecurity who also meet specific life situations and health conditions are eligible for this service.
