About this resource
This program is responsible for enforcing Oregon's workers' compensation regulations and overseeing funds that assist employers and employees. It offers guidance and directs inquiries to the relevant offices regarding workers' rights, compensation benefits, reemployment support, fraud reporting, dispute resolution, and ombudsman services.
Intake Procedure: Fill out Form 801, Report of Job Injury or Illness, and submit it to your employer. If you seek medical attention after your injury, complete Form 827, Workers and Health Care Providers Report for Workers' Compensation Claims, and mail it to PO Box 14480, Salem, OR 97309-0405.
