The program provides Maternity Support Services along with Infant Case Management. The Care Team is dedicated to supporting families by offering resources and education on pregnancy, breastfeeding, labor and delivery, parenting skills, and goal-setting. Services can be accessed via phone, at clinics, or in the comfort of the client’s home.
