Location: If heading north on I-5, take exit 271 (OR 214). At the traffic light, turn right onto Newberg Highway (OR 214) heading east. Then take a left onto Meridian Drive. The one-story building will be on your right.

- Individuals aged 60 and above, as well as those with physical disabilities aged 18 and over.
