This initiative involves public health nurses visiting homes to support families with children and youth facing special health care requirements. The home visitors aid families in managing care for their children, linking them to healthcare services and community resources, while ensuring collaboration among the child’s health care team. Additionally, it assists older youth with special health needs in preparing for the transition into adulthood.
