About this resource
This service provides extensive general medical care, vaccinations, mental health assessments and counseling, diabetes and sexually transmitted infections testing and treatment, breast cancer screenings, tobacco cessation programs, addiction evaluations and treatments, as well as support for Oregon Health Plan (OHP) eligibility.
Intake Process: Schedule an appointment by phone.
Fees: Available on a sliding scale.
Who can use this
- Available to Native Americans, Alaskan Natives, and other at-risk groups.

- Native American ID card, photo ID, birth certificate if direct descendent of enrolled Native American
