Who can use this

- Must have an HIV diagnosis - No active communicable Tuberculosis - Must qualify for Medicaid or possess private insurance covering necessary services - Must have a means to pay for room and board - Must meet income limits of 185% of federal poverty level guidelines - Requires 24-hour nursing care, demonstrated by any of the following: - Frequent hospitalizations or ER visits in the last 3 months - Difficulty managing a complex medication regimen - Changes in condition leading to new or worsening daily living activity deficits - A complex care plan due to unstable conditions or symptoms - Uncontrolled pain or symptoms like fatigue or nausea - Significant weight fluctuations - Severe dementia or behavioral challenges affecting the management of these issues.
