About this resource
This facility combines health care, housing, and employment services all in one location. The health care offerings encompass primary care, walk-in services, hepatitis C treatment, mental health support, acupuncture, and an on-site pharmacy. Most addiction treatment options are available, including suboxone, but methadone treatments are not provided. Additionally, transitional, supportive, and permanent housing services are offered.
Intake Process: Individuals can walk in or call to schedule an appointment. Help is available for those who qualify to register for insurance through the Oregon Health Plan or Medicare.
Travel/Location: Accessible via the MAX Blue line to 122nd St.
Fees: The facility accepts the Oregon Health Plan (including CareOregon and other Medicaid), Medicare, self-pay (with discounts for low-income patients), and some private insurance. No one is denied service due to lack of insurance or inability to pay.

- Individuals must be 18 years or older; eligibility criteria differ by program.
