About this resource
The offerings consist of regular physical examinations, sports physicals, treatment for illnesses and injuries, vision and hearing assessments, dental check-ups, blood pressure evaluations, immunizations, mental health support, age-appropriate reproductive health services, standard lab tests, prescription medication management, as well as education and promotion regarding health, fitness, and nutrition.
Intake Procedure: Please call to schedule an appointment.
Travel/Location: The health center has temporarily relocated to a mobile medical van situated on the west side of the Roosevelt High School campus, adjacent to the school building, due to ongoing renovations.
Fees: The center accepts Oregon Health Plan (OHP), certain private insurance plans, and offers sliding scale fees. No student will be denied services due to an inability to pay.

- Students from kindergarten through 12th grade.

- Insurance information, if applicable. Income information for those applying for sliding scale fees.
