This program provides various support services for individuals with intellectual and/or developmental disabilities, including case management, personalized support plans for individuals and families, access to transportation brokerage services, in-home assistance or residential options, as well as employment and vocational support.

- Individuals with an intellectual or developmental disability, both children and adults, who are residents of Oregon.

- Proof of address, and all records of psychological and medical assessments related to the disability
