This service delivers a broad spectrum of general medical care, including immunizations, mental health evaluations and counseling, diabetes testing, testing and treatment for sexually transmitted infections, breast cancer screenings, tobacco cessation programs, and assessments and treatments for addictions, along with support for Oregon Health Plan (OHP) eligibility.
Intake Procedure: Schedule an appointment via phone.
Travel/Location: Situated in the Goose Hollow neighborhood.
Fees: Offers a sliding scale for fees.

- Individuals who are Native American, Alaskan Native, or part of other vulnerable groups.
