About this resource
Delivers a broad spectrum of general healthcare services, such as immunizations, mental health assessments and counseling, diabetes testing, testing and treatment for sexually transmitted infections, breast cancer screenings, tobacco cessation programs, and evaluations and treatments for addictions, in addition to providing assistance with Oregon Health Plan (OHP) eligibility.
Intake Procedure: Schedule an appointment by phone.
Fees: Available on a sliding scale.

- Open to Native American or Alaskan Native individuals and other at-risk groups.
