About this resource
The clinic provides an extensive array of health services, such as physical examinations, treatment for minor injuries, immunizations, well baby and child clinics, nutrition counseling, mental health assessments and counseling, diabetes testing, testing for sexually transmitted infections, education and treatment programs, breast cancer screenings, tobacco cessation initiatives, addiction assessments and treatment, along with assistance for Oregon Health Plan (OHP) eligibility.
Intake Procedure: Schedule an appointment by phone.
Fees: Based on a sliding scale.
