ALERT serves as a statewide immunization registry that gathers vaccination information from both public and private healthcare providers, creating a comprehensive record for every person who has been vaccinated in Oregon.
Intake Procedure: To initiate a request, download the Request form from the ALERT website, fill it out, and send it via email, mail, or fax to the contacts provided at the top of the application.
Fees: There are no associated fees.
