About this resource
Provides various support services for individuals with intellectual and/or developmental disabilities. These services may encompass case management, personalized support plans for individuals and families, access to transportation brokerage services, in-home assistance or residential options, as well as employment and vocational training.
Intake Procedure: To begin the process, call to obtain an application for Developmental Disabilities services. An appointment will be arranged with an eligibility worker who will assist with the application.
Fees: There are no fees involved.
Who can use this
- Individuals of all ages with an intellectual or developmental disability, who are residents of Oregon and U.S. citizens.
Documents you may need
- Proof of citizenship, proof of address, and records of any/all psychological and medical assessments related to the disability
