About this resource
The program provides various counseling options, including individual, group, and family sessions, along with psychiatry services and medication management. It also includes advocacy, case management, substance use disorder counseling, and crisis intervention. Additionally, the Connection program caters specifically to deaf or hard of hearing individuals and their families. Services are accessible through remote video conferencing, email, and instant messaging, along with referrals for housing assistance, emergency food, and employment resources.
Intake Procedure: A referral from a primary care provider at West Salem Clinic or Total Health Community Clinic is required.
Fees: The program accepts Oregon Health Plan (Medicaid), Medicare, Tricare, most private insurance, and provides a sliding fee scale based on income and family size for those without insurance or with limited insurance coverage. No one is denied service due to an inability to pay.
