About this resource
This program delivers in-home care services such as housekeeping, personal assistance, laundry, shopping, transportation, and meal preparation and delivery. It also provides adult day services, which are community-based programs that operate for part of the day. Registered nurse services are available for one-time or periodic assessments, service planning, medication management, and provider training and education. Additionally, case management and options counseling are offered to aid in informed decision-making regarding services based on individual strengths, needs, and preferences. The number of service hours provided varies based on assessed needs and available funding, and benefits are not subject to estate recovery.
Intake Procedure: Interested individuals can call or visit the website for further details. Some services may have a waitlist.
Travel/Location: If traveling north on Highway 101, turn right onto McCormick Loop Road, then left onto Third Street at the end of the road. The facility is a one-story tan building on the right, opposite the Forestry Department. If heading south on Highway 101, turn left at the traffic light after the Tillamook Cheese Factory onto Wilson River Loop Road. At the first stop sign, turn right (it remains Wilson River Loop Road). Proceed past the blinking red light at the Highway 6 intersection and continue uphill, then turn left at the stop sign onto Third Street.
Fees: There are no fees associated with the services.
Who can use this
- Eligible participants are adults aged 18 and older who require assistance with personal care, have an income not exceeding 400% of the Federal Poverty Level, possess assets below the average cost of six months in a nursing facility, and reside in their own home without being part of a provider-operated or owned facility or foster care home.
