This initiative features public health nurses (PHNs) who conduct home visits to collaborate with families in enhancing health outcomes during pregnancy, for families, and for children. The program offers health and developmental screenings, education on health and nutrition, guidance on infant and child development, breastfeeding assistance, and connections to community resources tailored to the family's specific needs.

Who can use this
- Eligible participants include perinatal individuals, infants and children aged 0-5, and parents or primary caregivers facing higher health risks, such as prematurity, teenage parenthood, at-risk caregiving, domestic violence, or drug exposure.
