About this resource

This program delivers in-home care services encompassing housekeeping, personal assistance, laundry, shopping, transportation, and meal preparation and delivery. It also provides adult day services, which are community-based programs that operate for part of the day. Registered nurse services include one-time or periodic assessments, service planning, medication monitoring, provider training, and education. Additionally, case management and options counseling are available to aid individuals in making informed decisions regarding services tailored to their strengths, needs, and preferences. The number of service hours per individual is determined by assessed needs and funding, typically ranging from 5 to 20 hours monthly. Importantly, benefits will not be subject to estate recovery. For intake, individuals can call or visit the website for further details, noting that some services may have a waitlist. The location can be accessed through the front lobby doors on SE Byers Avenue, located on the south side of the building. There are no fees associated with these services.

Who can use this

  • Adults aged 18 and over who require assistance with personal care tasks, have an income not exceeding 400% of the Federal Poverty Level, possess assets below the average cost of six months in a nursing facility, and reside in their own home.
