This service offers a variety of support for older adults and individuals with disabilities, such as assessments, case management, and the management of SNAP (food stamps), Oregon Supplemental Income Program (OSIP), other Medicaid initiatives, Medicare Savings Programs, along with information and referrals to coordinated services.
Intake Procedure: You can either walk in or call for additional information. Certain programs may necessitate an in-person or phone interview appointment.
Fees: There are no fees involved.

- Individuals aged 60 and above and adults with disabilities aged 18 and older.
