About this resource
The program offers a comprehensive array of services that may encompass networking and outreach, leading Individual Support Plan (ISP) meetings, aiding in the referral process to community service agencies for necessary support, and assisting individuals and families during various planning meetings. Additionally, the program helps in locating residential placements and employment services, as well as making referrals to support service brokerages. Staff may also investigate complaints of abuse and take immediate measures to evaluate any potential risks. In certain situations, protective services might be provided to prevent further harm.

Who can use this
- Applicants must meet all of the following criteria: - Have a brain-originating disability - The disability should be established before age 22, or for intellectual disabilities, before age 18 - Must have significant impairments in at least two areas of daily functioning, including self-care, communication, cognition, mobility, self-direction, independent living capacity, or economic self-sufficiency - Impairments should not be primarily due to substance abuse, mental illness, emotional disorders, ADHD, learning disabilities, or sensory impairments, and must be expected to persist indefinitely.
