This program offers a wide array of physical, mental, and dental health services. These services encompass routine examinations, well-child check-ups, vaccinations, treatment for illnesses or infections, prenatal care, extensive dental services, as well as counseling and treatment for drug and alcohol issues.
Intake Procedure: Schedule an appointment by phone.
Fees: No charges.
Who can use this
- Enrolled members or descendants of federally recognized tribes who register as patients; Purchased & Referred Care has residency rules.
Documents you may need
- Tribal enrollment card with photo, Certificate of Indian Blood, or proof of Native descent; completed registration application; alternate insurance enrollment if eligible
