About this resource
This program delivers confidential case management and independent living support for individuals diagnosed with HIV. It includes financial assistance for HIV medications, transportation to medical appointments, insurance premiums, transitional housing, rent support, dental care, disease management education, and referrals to other organizations for basic necessities, employment, and substance abuse treatment.
Intake Procedure: Schedule an appointment via phone.
Fees: There are no fees associated.
