This program delivers confidential case management and independent living support to those living with HIV. It includes financial assistance for HIV medications, transportation to medical appointments, case management sessions, insurance premium payments, transitional housing, rent support, dental care, disease management education, and referrals to other agencies for basic needs, employment, and substance abuse treatment.
