About this resource
Provides a variety of support services for individuals facing intellectual and/or developmental disabilities. These services may encompass case management, individual and family support plans, access to transportation brokerage services, in-home assistance or residential placement, as well as employment and vocational support.
Intake Procedure: To initiate the process, call to obtain an application for Developmental Disabilities services. An appointment will be arranged with an eligibility worker who will assist you throughout the application process.
Fees: There are no charges for these services.
Who can use this
- Individuals with an intellectual or developmental disability must be Oregon residents and U.S. citizens.
Documents you may need
- Proof of citizenship, proof of address, and records of any/all psychological and medical assessments related to the disability.
